The Department of Health (DH) has set up the Centralised Organ Donation Register (CODR) to make it more convenient for prospective donors to voluntarily register their wish to donate organs after death, and for such wish to be more reliably recorded. The Register will enable medical personnel responsible for organ donation to know upon the patients' death about their wish to donate organs, and the bereaved family to acknowledge the deceased's wish to rekindle lives of other people.
To sign up
- Register online; or
- Collect or download the registration form in the organ donation promotional leaflet and send it after completing the details
- by post to:
CODR System Administrator, Department of Health, 21/F, Wu Chung House, 213 Queen's Road East, Wan Chai, Hong Kong; or
- by fax to 2127 4926
Attention
- Upon receiving your organ donation form, DH will contact you by telephone to verify your personal particulars. DH will issue a new form to you by post or via email using the address you provided if she fails to establish contact with you.
- If you do not provide the requested information, DH may not be able to complete the registration process for you.
- If you have already registered with the CODR and would like to update your particulars or withdraw your registration, please re-submit the registration form or complete the withdrawal form online, by post or by fax. DH will call you to update your record or remove your registration after verifying your personal particulars.
Organ Donation Card
The organ donation card is designed for those potential donors who prefer not to register in the Centralised Organ Donation Register. They should tell their family about their wishes and carry the completed Organ Donation Card at all times.

The Organ Donation Promotional Leaflet and Organ Donation Card can be collected at the reception of the following distribution points :
